During my prior practice at Fairview Ambulatory & Chronic Care Clinic, I focused on longitudinal management for patients presenting with complex respiratory diseases, hypertension, and endocrine disorders. While there, I implemented an outpatient chronic disease monitoring protocol that reduced related emergency department visits among clinic roster patients by 18 percent. I also conducted specialized polypharmacy reviews for elderly clients to eliminate duplicate therapies and mitigate adverse medication interactions, collaborating closely with regional health authorities and allied health partners to coordinate safe home-care transitions.
